• Comprehensive in-home assessment with personalized written action plan

• Fall prevention and home safety recommendations

• Simple daily routines that reduce confusion

• Hands-on caregiver guidance

• Safe, practical movement and exercise recommendations

*** With in 15 mile radius of home location. Additional distance costs:

  • 16-25 miles: $25

  • 26-40 miles: $50

  • 41+ miles: Negotiated rate

Ongoing Support and Coaching- Virtual or In-Home
$60.00

• Virtual follow up. 30 minute virtual consult to follow up on action plan

• Caregiver training and support

• Insurance and financial guidance

• Home environment optimization

• Coordinating of community and Medicare services/benefits
